Mind and Memory Partners is a community-based 501(c) (3) nonprofit organization dedicated to enhancing the lives of individuals living with brain changes and the care partners who support them. Through education, meaningful engagement, and community connection, we help families navigate the challenges of brain change while promoting brain health, early awareness, and cognitive wellness.
We envision an inclusive and caring community where stigma surrounding brain change is replaced with understanding and compassion, cognitive health is nurtured, and individuals and care partners are supported, connected, and never alone.

The Memory Café offers a welcoming, dementia-friendly environment where individuals living with cognitive changes and their care partners can connect with others, participate in enjoyable activities, and build meaningful relationships.

Caregiver Respite & Recognition Program
The Caregiver Respite and Recognition Program provides a one-time monetary gift* and care package to unpaid family caregivers of individuals living with dementia or other neurological conditions.
The gift may be used at the caregiver's discretion for respite, personal wellness, household assistance, transportation, meals, or other needs that help them recharge and continue providing care.
Goals:
Prevent caregiver burnout
Promote self-care
Recognize and honor family caregivers
Improve caregiver well-being
*Monetary gifts of $500 are awarded to one caregiver on a quarterly basis. All caregiver applicants will receive a care package.
*This new program will be funded through generous donations.

Community Education
Mind and Memory Partners provides educational presentations and outreach programs for community groups, faith communities, businesses, healthcare organizations, and the public.
